ENZO  Building and Design, LLC requires that all subcontractors fill out and submit the pre-qualification form in order to be eligible to contract work with our company.

All Subcontractors interested working with  ENZO Building and Design, LLC are required to provide the following:

Subcontractor Documents (See PDF’s below)
   • SUBCONTRACTOR PRE-QUALIFICATION APPLICATION
   • W9
   • RELEASE OF MECHANIC’S LIEN

Before you start work on the job site, please return:
   • Subcontractor Pre-Qualification Application – fully executed
   • Insurance Certificate- following the specifications and limits as described in “Insurance, Workers Compensation, and Waiver Requirements for All Subcontractors”
   • Under description of operations (or on a separate page), you must add the ‘additional insured’ language
   • Once you submit a certificate with this language, it will cover all jobs. Therefore, do not list a job name
   • Worker’s Comp Certificate (must show what state it covers)
   • W9 – Required if this is your first time working with ENZO Building and Construction
